Documents Needed:

Please submit the following documents ALL TOGETHER. You can email them directly to

  • Proof of residence-current utility bill (gas or electric), mortgage statement or lease agreement from the last 45 days,
  • Parent/guardian photo ID (must match the name on the proof of residence and birth certificate)
  • Proof of guardianship if applicable,
  • Birth certificate, 
  • Up-to-date Immunization record or notarized exemption,
  • Report card/withdrawal paperwork from previous school,
  • Court documents pertaining to your student (if applicable)
